It has come to my attention that parents will also need to include a copy of their photo ID with the Mega Cavern Permission form since parents will not be present. You may omit the parent’s weight and height. Please include this with the forms when they’re turned in. We can make a copy of your license while at church for anyone who does not have access to a copier.
