Fees and Forms Schedule

Your players should be aware of which tournaments they are attending.  If there are any questions, please let us know.  Please read post all the way through!

Tournament fees for players only are due to us on Monday, April 18th, as follows:

• Milligan – $60
• KCU – $100
• Lincoln (Reagan, Geoffrey, Jake) – $80
• Bristol
  • Team 1 (registration fees should have been paid to Emily Haise): $60 (hotel and transportation fees)
  • Jacob, Jackson, and Jonathan: $90

The following people need to submit the following forms to us by Monday, May 2nd:

• All students traveling:
  • MEDICAL CARE POA Bible Bowl Student Trips – SpringSummer 2016 (must be notarized)
  • 1 copy of insurance card per family to accompany the POA
• All students going to Nationals:
  • National Bible Bowl Parental Consent Form
• All students going to KCU:
  • Individual-Registration-Form-June
  • Permission-Medical-Form-Students
  • Code-of-Conduct-Form
• All sponsors going to KCU:
  • Code-of-Conduct-Form
  • Medical-Release-Form-Sponsors

$450 needs to be submitted for plane tickets to/from Nationals for the following people by Monday, May 23rd:

• Bill Schneider, Beth Schneider
• Craig Daugherty, April Daugherty, Lucy Daugherty
• Steve Haise, Emily Haise, Ryan Haise
• Kevin Daugherty
• Adelyn Rolston

$111 (the price went up, ugh!) needs to be submitted for Disneyland tickets for the following people by Monday, May 23rd:

• Bill Schneider, Beth Schneider
• Craig Daugherty, April Daugherty
• Steve Haise
• Kevin Daugherty
• Buddy Dowdy, Maronda Dowdy, Madison Dowdy
• Tracy Murray

$105 needs to be submitted for Disneyland tickets for the following people by Monday, May 23rd:

• Lucy Daugherty
• Adelyn Rolston
• Dallas Dowdy
