We mentioned in our Summer Tournaments post about needing a permissions form completed by parents. You can now download that form at MEDICAL-CARE-POA-Bible-Bowl-Student-Trips-Summer-2018. This form needs to be notarized, so please make sure to sign it in front of a notary. Usually your bank has someone on staff who is a notary, Southeast may as well. Please turn in these forms with your Nationals fees.
