Parental Consent Form Needed ASAP

We need to submit Nationals paperwork by the end of the month, and required for each player is the parental consent form found here: http://www.biblebowl.net/Data/Nationals/2012_NatsTeenPlayerParentalConsentForm.pdf

Please print, fill out, and send with your player to practice on Monday.  If you are unsure as to whether your child is attending nationals, FILL ONE OUT ANYWAY – better to send it and not need it than vice versa.  We will have some printed out at practice if you’d whether come in and fill one out then.  Thanks for your prompt response!
